Often, people may not notice it until they start to do an activity like running. </span><a href="https://my.clevelandclinic.org/health/articles/14611-kegel-exercises" title="Kegels" rel="" style="text-decoration-line:underline;color:rgb(48, 4, 234);">Kegels</a><span style="color:inherit;"> are the first treatment often employed but if symptoms are not reducing, we need to look further and deeper into the issues.&nbsp;</span><span style="color:inherit;">Because running involves whole body movement, we then MUST look at the movement itself. Doing kegels lying down or while waiting at a stop light might not cut it in solving your issues so p</span><span style="color:inherit;">ay attention to if any of the following is happening:</span></p><p style="color:inherit;"><span style="font-weight:bold;color:inherit;"><br></span></p><p style="color:inherit;"><span style="font-weight:bold;color:inherit;">Breathing</span></p><p style="color:inherit;">How are you breathing while you run? Are you a nose or mouth breather? Do you tend to breathe into the chest, mid chest or ribcage, or into the belly?</p><p style="color:inherit;"><span style="color:inherit;"><br></span></p><p style="color:inherit;"><span style="color:inherit;font-weight:bold;">Abdominal muscles</span></p><p style="color:inherit;">Over-gripping of the abdominal muscles can lead to increased downwards pressure on the pelvic floor. If you tend to engage your core with physical activity, try decreasing how much you engage it and focus on getting nice long inhales and exhales instead.</p><p style="color:inherit;"><span style="color:inherit;"><br></span></p><p style="color:inherit;"><span style="color:inherit;font-weight:bold;">Hip Weakness</span></p><p style="color:inherit;">Are your hips able to support your frame as you are running? The glut bum muscles in the back and the hip flexors in the front are huge muscles that propel you forward, working alongside the knee muscles. Unresolving leakage may involve the hip or knee muscles and functionally training these would be key.</p><p style="color:inherit;"><span style="color:inherit;"><br></span></p><p style="color:inherit;"><span style="color:inherit;font-weight:bold;">The Pelvic Floor</span></p><p style="color:inherit;">Functionally training the pelvic floor for the task it is a part of is a very important part of rehab. If you are doing kegels lying down, they might not transfer to running and jumping. Doing kegels progressively harder and harder trains the muscles to know what to do when you are running so you don’t have to think about doing a kegel when you run. Try doing kegels in standing, with squats, lunges, walking, and finally jumping.</p><p style="color:inherit;"><span style="font-weight:bold;color:inherit;"><br></span></p><p style="color:inherit;"><span style="font-weight:bold;color:inherit;">Running Posture</span></p><p style="color:inherit;">Running with your chest up and out is something you may have been told to do in your high school cross country club, but this can lead to increased pressure on the pelvic floor. Although Physiotherapy schools are great in providing the foundational knowledge for treating the body externally, the pelvic floor is the missing piece of this education. That is why there is a scarcity of Pelvic Floor Physiotherapists. We take post-graduate courses to further our training and even then, only a small percentage decide to practice in this area.</span></p><p><span style="color:rgb(45, 11, 11);"><br></span></p><p><span style="color:rgb(45, 11, 11);">But, what do we actually DO? Your Pelvic Floor Physiotherapist has been trained to assess the back, hips, pelvis, and abdomen from the eyes of how it functions with the bowel, bladder, vagina, rectum and also treat low back/hip issues that are not resolving. We do this by looking at everything outside and then also performing internal examinations to see how the muscles that control urination, sexual function and bowel movements are doing.</span></p><p><span style="color:rgb(45, 11, 11);"><br></span></p><p><span style="color:rgb(45, 11, 11);">Internal exam? Yes. The only way to know how your pelvic floor muscles are, in terms of strength/weakness and also “tightness/looseness” is by providing you with that information and feedback. In this article, we outline what an assessment looks like and what treatment options might be used.</span></p><p style="color:inherit;"><br></p><p style="color:inherit;"><span style="font-weight:bold;">Medical History</span>:&nbsp;</p><p style="color:inherit;">Your Pelvic Health Therapist will take a complete history of your symptoms including when they started, when they bother you, your medical history and your goals for physiotherapy.</p><p style="color:inherit;"><br></p><p style="color:inherit;"><span style="font-weight:bold;">External Assessment</span>:&nbsp;</p><p style="color:inherit;">As the pelvic floor muscles work with your hips, back, and abdomen, all these muscle groups may be assessed dependent on your concern. The testing may be getting you to do certain movements and testing for muscle strength to test both strength and flexibility.</p><p style="color:inherit;"><br></p><p style="color:inherit;"><span style="font-weight:bold;">Internal Assessment</span>:&nbsp;</p><p style="color:inherit;">Your examination MAY include an internal component, depending on your symptoms. When this may be deemed appropriate, the physiotherapist will discuss with you thoroughly how the examination will be performed and what will be tested. Most of the time patients are wanting to have this examination so they know what is causing their symptoms. However, if you do not feel comfortable, please note that the internal component is not a requirement and will not be performed without your consent.</p><p style="color:inherit;"><br></p><p style="color:inherit;"><span style="font-weight:bold;">Assessment Findings:&nbsp;</span></p><p style="color:inherit;">The physiotherapist will verbally tell you of their assessment findings and based on this will provide you with treatment, which may include any of the following:</p><p style="color:inherit;"><span style="font-style:italic;text-decoration-line:underline;">manual therapy</span>: stretching and massage techniques performed by the physiotherapist</p><p style="color:inherit;"><span style="font-style:italic;text-decoration-line:underline;">strengthening and stretching exercises</span>: these will be homework for you to perform at home</p><p style="color:inherit;"><span style="font-style:italic;text-decoration-line:underline;">education</span>: certain tips to alleviate the symptoms you are feeling</p><p style="color:inherit;"><span style="font-style:italic;text-decoration-line:underline;">Questions</span>: Any questions you may have will be addressed at this point and further information will be provided</p><p style="color:inherit;"><span style="font-style:italic;text-decoration-line:underline;">Treatment Planning</span>: based on your symptoms and presentation, the physiotherapist will guide you towards how often to come in, what further treatment will look like and that they will email you the exercises to follow along with at home.</p><p style="color:inherit;"><br></p><p><span style="font-weight:bold;"><span style="color:inherit;">And, that’s it!
